Reroot (https://www.reroot.care) is a specialised MDT (~ 10 different specialists in integrated mental & physical health) led by Clinical Director who mentors & supervises the team in illness treatment and recovery. It is an outpatient polyclinic, community & home healthcare provider.

Reroot is in favour of translational research and has embedded 34 streams of clinical care delivery extracted from current research investigations contributing to better outcomes in mental health. We are at the forefront of such thorough & holistic care delivery for mental illness recovery in India.

Less is known by people that Mental & Physical Health are very interlinked and there is an urgent need to simultaneously address both aspects together for better outcomes. WHO, America, Europe have for some time declared there is no health without mental health. The bidirectional impact of physical and mental health needs an integrated set up. On site, telemedicine, rehabilitation in the community, and home healthcare is delivered for both mental & physical health.

Domiciliary assessments in the vicinity are provided for limited mobility, urgent assessments, or clients refusing assessments where concern is raised by their carers, in the absence of medical emergencies. Early detection, early intervention, good client engagement, medication management, psychoeducational engagement, is the hallmark of recovery or stability from mental disorders and chronic medical conditions. QoL improving recommendations are provided to clients & their families for chronic limited recovery potential disorders. Realistic & humane palliative care is ensured from Reroot. We will endeavour to overcome challenges of community & home care & healthcare, providing training/supervision/monitoring/mentoring to these areas of staffing,

Both physical & mental healthcare will be provided in the community, and at client’s house by our team outreach. This will include chronic, palliative, end of life, disability, limited ability, differently abled needs clients, and clients needing rehabilitation care at home or in local community. We have structured the team, and customised training provided to care assistants, health care assistants, nurses, to improve safety and quality of home care and community care. In collaboration, we provide home care assistance for various domestic or family needs, & home health care assistance from Reroot is delivered in a tiered approach from healthcare assistants (www.amarjyothi.online), nurses, visiting specialists including doctor; monitored and trained by senior doctor & clinical director. Multiple specialists for mental & physical health will also provide outreach in the community & at home under monitoring/mentoring as per the requirements of the client or their families.

This improves access, reduces carer burden, improves early detection and screening, health worker informed healthcare management, and reduces logistic burden to families. Chronic medical and mental healthcare becomes more meaningful and transparent, allowing early planning & decision making as per the course of requirements. Body fluid samples can be collected at home, and Health care assistants can accompany and arrange transport of client to nearby centres or hospitals for investigations & follow ups. When patients are discharged from hospitals, continuity of recommended medical care by the discharging doctor from hospital, can be provided at home with our outreach staff if medical condition is stable. Home care will not include first time home diagnosis or initiating new treatments, unless it is safe to do so at home. Medical emergencies will be referred to your chosen hospital, and transport assistance to nearby chosen hospital for elective admission will be supported.
